ACTivist In-School Visit Request Form

Thank you for your interest in the Missouri History Museum's ACTivist In-School Visit program!

NOTE: If you're looking to schedule an on-site visit to MHM, please visit mohistory.org/fieldtrips.

All schools requesting an ACTivist visit must complete and submit this form at least 2 weeks before the date of your desired performance to secure a reservation.

Please understand that your visit isn't finalized until you receive a confirmation letter via email.

ACTivist in-school visit

*One performance can accommodate a maximum of 40 students.  Please indicate the number of performances you will need to accommodate your students (Example: 92 students would require 3 performances.)

Please indicate three dates that would work for your school. It is recommended that an ACTivist visits your school before a field trip to the Museum.

The grant which supports this offering does not extend to summer-time visits, but we are accepting requests for 8/28/17 through 3/30/18.

Please be aware that no performances are guaranteed until confirmed by our Reservations Department. Dates and times are dependent upon the availability of the actors, and availability is very limited. (If you'd like to add more potential dates than the three indicated above, feel free to do so in the comments box below prior to submitting this request!) We can only make in-school visits to locations within 20 miles of the Missouri History Museum.
